Adding a custom gender field to a digital waiver

To add a custom gender field to a digital waiver, you’ll want to first create a custom text field in your customer profiles for gender. You will then create the custom field; when the user makes a selection, that choice will then show up in their customer profile.

First, let’s create the custom text field option for customer records in the Settings.

  1. In the Data Entry main window click Manage and select Settings to bring up the Configure Settings window.
  2. Click the Waivers/Custom tab.
  3. Under Custom Policies/Skills/Text check Use Custom Text Field 1 or Use Custom Text Field 2. Enter “Gender” for the field.
  4. Click Close to finish. 

Next, let’s create the field in the waiver.

  1. Back in the Data Entry main window click Manage and select Waiver Management / Stations to open the Waiver Management window.
  2. Select the form you wish to add the gender field to and click the Edit button.
  3. Click the Step 2: Edit Form tab.
  4. Under Form Sections click the Add button to open the Edit Section window.
  5. Select the Field Group section type. For Group Label use “Gender”. Click OK.
  6. With the gender form section selected, select Choice List from the drop down menu at the bottom of the screen and click Add.
  7. In the Edit Form field fill in the following fields:
    • Customer Target: Select Gender. This will now store the information in the customer record.
    • Required: Check the box to make this information required. The form will not be accepted if this field is left blank.
    • Choice List: Enter Male and Female. Put each choice on its own line.
  8. Click OK to save the text field. If the system creates a default value of Male or Female click Edit to go back in and erase the default value. 

Finally, add the code to the waiver to indicate we want it to use the field.

  1. Click Step 4: Edit Document.
  2. Go to the Table Markup (Between STARTTABLE and ENDTABLE and enter [Gender] {{gender}} (similar to the other markup language) where you want this field to display in your form.

Click OK to save. Once you’ve reviewed the waiver in design mode click Publish Design To Live to make your edits go live.
