Optional Element on Electronic Waiver

Our gym requires a use of a helmet for all individuals unless they've signed a helmet waiver. We record this in RPG as our second policy. For our paper waivers, the Helmet Waiver portion appears as another section of the Main Wocument that can optionally be signed. Since optional signatures aren't available in RGP. I was hoping you might have a suggestion how we should do this, but I've tried to come up with some ideas how we can represent that in electronic waivers.

I understand that these ideas would require extra work on the staff person's part to update the customer record properly.

Option 1. Add a signature line where we have the climber write out a sentance such as "I must wear a helmet" or "I may participate without wearing a helmet" rather than an actual signature. This option would work pretty well, but it still says "SIGNATURE" on the form - so that might lead to confusion and a rejected waiver when we hand a helmet to someone who signed this spot instead of writing the correct text. Still, this may be our best option.

Option 2. Create a print&sign waiver. This seems like a lot of extra paper simply to allow someone to optional sign something.

Option 3. Separate both documents and create a Helmet Waiver document that people would be directed to upon filling out the Main Waiver document. However, this would also require people to enter a lot of duplicate information and seems like a lot of extra steps.

Option 4. Create the Main Waiver document and a separate Main Waiver + Helmet Waiver combined document and have people choose before filling out anything. It would require people to know what document to start with before they knew _why_ they might need to choose.

Any best practices here?

  • Why do you require helmets unless explicitly waived? Is that a condition imposed by your insurance company? Coming from an area where helmets are rarely seen in indoor gyms, that's surprising to me.

    Our insurance company was content with this as part of our waiver:

    I acknowledge that TNC has helmets available for all participants at no cost, and that it is my responsibility to ask for a helmet if I choose to wear one. By not choosing to wear a helmet, I am voluntarily assuming any additional risks of injury this may cause.

    or for the minor waiver:

    I acknowledge that TNC has helmets available for all participants at no cost, and that it is my responsibility to ask for a helmet if I choose to have my child wear one. By not choosing to have my child wear a helmet, I am voluntarily assuming any additional risks of injury this may cause.

    There is a spot to initial very close to this text.

    If the policy is imposed by your insurance company, perhaps they would be willing to relax it enough to allow you to adopt this approach and simplify your waiver process.

    For anyone search for something similar, here's an excerpt from the waiver Andy suggests:

    It is my responsibility to request a helmet if I choose. please sign to make helmet use optional or write "must wear helmet"
