Creating a Yes/No field in a waiver form

A Yes/No field can be added to a waiver form to help capture consent. Here we’ll look at the settings you’ll need to make to create a Yes/No field.

  1. To get started, create a new text field, selecting Yes/No from the drop down menu..
  2. In the Edit Form field fill in the following fields:
  • Customer Target: Select where you would like this information to be stored in the customer record from the dropdown menu if applicable
  • Document Field: Enter the placeholder field name you would like on the document. The name must be formatted with double brackets like so: {{experience}} {{belaycertification}} {{newsletter}}
  • Is Required: Check the box if this information is required. That way the form will not be accepted if this field is left blank.
  • Default Value: Enter either Yes or No, depending on which one you would like to be the default
  • Choice List: Enter “Yes” and “No” as the two choices. Enter “Yes” first.
  • Click OK to save the text field.
  • Click OK to save the form and then Close to exit the Waiver Management window.
