Creating a choice list field in a waiver form

A choice field can be added to a waiver form to capture information that has limited options such as a gender, local school or experience range. Here we’ll look at the settings required to create a choice field.

  1. To get started, create a new text field, selecting Choice List from the drop down menu..
  2. In the Edit Form field fill in the following fields:
  • Customer Target: Select where you would like this information to be stored in the customer record from the dropdown menu if applicable
  • Document Field: Enter the placeholder field name you would like to use on the document. The name must be formatted with double brackets like so: {{gender}} {{yearsofexperience}} {{highschool}}
  • Required: Check the box if this information is required. That way the form will not be accepted if this field is left blank.
  • Default Value: Enter which choice you would like to be the default if applicable
  • Choice List: Enter the choices you wish to display. Put each choice on its own line. Check Include “Other” Text Box if you wish to have “Other” as an option; if selected this will display a text field the participant can enter information into.
  • Click OK to save the text field.
  • Click OK to save the form and then Close to exit the Waiver Management window.
