I just pulled up the first page of all your waivers forms on my iPad without issues. If the URL you are using was incorrect, that would produce the message you are describing.

All family members will show one guest pass available. The logic is checked at consumption. So on the second guest pass used by the family member in a month, there will be a warning/stop.
